~ New Patients ~

We are accepting new patients at this time, however we do have some stipulations.

We are not accepting new patients coming to us due to:

· an on-the-job-injury or workers compensation claim,

· a motor vehicle accident,

· a disability claim,

· are on VA

· or any new patients that are currently taking chronic pain management medication.

We now require new patients to fill out a New Patient Application prior to making any appointments, unless there are extenuating circumstances.

If you are currently on any prescription medications and would like to establish care with one of our providers, please call the office and discuss this with a nurse or medical assistant. If you have any questions about whether or not you would be accepted as a new patient, please arrange to get your records transferred to our office for review and one of our staff will be in contact with you as soon as possible. You can call the office to have the form mailed or faxed to you. If you would like to initiate the transfer with your current provider, they will have their own forms for you to sign.

Before your first office visit, please contact your insurance company to find out if they will cover a visit to one of our providers. If you do not currently have insurance, payment is due at time of service. Our office offers a 15% discount for payment at time of service.

Payment of co-pays or deductibles are due at time of service.

Please arrive at least 15 minutes prior to your appointment time to complete your paperwork. If you would like to get started before your appointment, you can print a copy of the registration form to complete and bring to your appointment with you.
